The rough-in valve can be installed in any direction as long as the bathtub center line is aligned with one of the six screws on the plaster guard. The plaster guard has a flat top for easy leveling in installation to ensure the trim will be standing straight after installation. However, there's no mention or photo that shows this part is only the 'sleeve' that is inserted into the rough-in mix valve - it doesn't include the internal cartridge (must purchase separately if needed).
