You need a moisture barrier between your baby's wetness (diaper accidents or spit up) and the mattress to prevent such things as mold and smell to name a couple. Don't think it's a pad like your adult mattress padding you buy in a mattress store. A baby's mattress should be as firm as possible while their time in the crib and they don't need a hotel type feel for bedding. This is to prevent such things as I have previously mentioned from happening to the mattress itself and extending the life of the mattress. It doesn't have elastic or straps to hold onto the mattress but putting sheets over it isn't a problem unless your all thumbs.
