American Standard Hampton Bath Tub and Shower Faucet with Porcelain Lever Handle

We used this to replace an old 3-handle shower faucet by getting 3" hole covers to cover over the old holes. Be warned, though - if you want to do this kind of project and have an old faucet with the modern standard 8-inch spread, you'll need to use a cover plate instead, and I'm not sure if there's a generic one you can use with this faucet. Our old faucet was 11.5 inches, so we had plenty of room to put this in the center hole and cover over the other two holes, and it looks much better! We are going for a classic art deco but modern look, and this works perfectly. Plus it's modern technology - no more washers!
